Inclusive Connection Care provides a comprehensive range of NDIS support services across Brisbane and Moreton Bay, Queensland. Our five core service areas are: Individual Support Services — Personalised daily assistance including personal care, household tasks, medication support, skill development, and community participation. All supports are delivered by trained, compassionate workers matched to each participant. Respite and Short Term Accommodation (STA): Quality respite care for NDIS participants that gives families and primary carers a well-earned break, with confidence that their loved one is receiving attentive support in a safe environment. Supported Community Access: One-on-one support to help participants attend social events, access community services, join recreational programs, and build meaningful connections with their local community. Support Coordination: Dedicated coordinators who help participants understand their NDIS plan, connect with the right providers, and build the capacity to manage their own supports with confidence over time. Supported Independent Living (SIL): Daily in-home or shared-living support for people with disability who want to live independently, with skilled workers assisting around daily tasks, personal care, and community integration.

Support Coordination is an NDIS-funded service that helps you understand your NDIS plan, connect with the service providers who are right for you, and make the most of your funding. A Support Coordinator acts as a knowledgeable, proactive guide who works with you to implement your plan, resolve issues with providers, and prepare you for your next NDIS plan review. Not every NDIS participant has Support Coordination funding included in their plan — it depends on your individual circumstances and the decisions made at your planning meeting. However, if you have complex support needs, multiple providers, or are new to the NDIS, having a Support Coordinator can be genuinely transformative. ICC Support Coordinators are experienced in navigating the NDIS and are committed to building your own skills and confidence so that, over time, you become increasingly independent in managing your own supports. Supported Independent Living, commonly known as SIL, is NDIS-funded support that enables people with disability to live as independently as possible in their own home or in a shared living arrangement. SIL funding covers the cost of support workers who provide assistance with daily tasks, personal care, meal preparation, household management, medication prompting, and participation in the wider community. SIL is generally designed for NDIS participants who need regular or around-the-clock assistance to live safely. It is one of the more significant categories of NDIS funding and requires a detailed assessment of your support needs to determine the appropriate funding level. At ICC, our SIL services are built on the belief that independence looks different for everyone. We work closely with each participant to understand their goals and gradually build their skills and confidence over time.
